What our forms collect

The group intake form asks for your first name, your email, which group you are joining, which of the three conditions you live with, a five-question wellbeing check-in (the WHO-5), and your agreement to the group agreements. County and income band are optional and used only for anonymous reporting to funders.

The volunteer form asks for your name, email, optional phone, availability, and a description of your lived experience.

The contact form asks for your name, email, and message.

Every form has a hidden field that only automated bots fill in. If it is filled in, the submission is discarded.

Where it goes

Form submissions are stored in a private database (Supabase) that only Rebuilt Minds staff can read. The website itself can write to that database but cannot read from it.

Zoom handles registration for the meetings and sends you your join link. Zoom's own privacy policy governs that step.

Group sessions

Group sessions are never recorded. Not by us, and, by group agreement, not by anyone in the room.

HIPAA

Rebuilt Minds is a peer support organization, not a health care provider. We are not a HIPAA covered entity. We treat what you tell us as if we were.

What we do not do

We do not run analytics or tracking scripts on this site. There is no Google Analytics, no Facebook pixel, no advertising cookies.

We do not sell, rent, or share your information with anyone. Anonymous, aggregated counts (for example, how many people joined a group in a quarter) may appear in grant reports.

Deleting your information

Email hello@rebuiltminds.org from the address you used and ask. We will delete your records within 30 days and confirm when it is done.
